What does SNALR stand for?

SNALR stands for Special Needs Assisted Living Residence, New York's certification for assisted living that serves people with dementia or other cognitive impairment under a plan the Department of Health approves.

It is an add-on to an Assisted Living Residence license, counted in beds.

Are all memory care units in New York locked?

Not necessarily; many SNALR units use secured or delayed-egress doors, but the certification is defined by the approved special needs plan rather than by one physical feature.

Ask how the unit keeps residents safe and how they get outdoors.

Can a resident with dementia stay in a regular assisted living apartment?

Yes, as long as the residence can meet their needs safely and they do not require a specialized dementia setting; once they do, New York requires a SNALR bed.

Many residents start in an ALR bed and move to the SNALR wing later.

How many memory care beds are there around Rochester?

The Department of Health's capacity report shows the SNALR bed count for every residence; our memory care directory adds them up for the five counties and lists each one.

Bed counts change when residences add or drop certification.

Do nursing homes have memory care units?

Many do, but New York's bulk nursing home data does not report dementia unit sizes, so the only way to know is to ask each home how many beds its dementia unit holds and how it is staffed.

Nursing home dementia care is the main option for people who also need skilled nursing.

What questions should I ask a memory care residence?

Ask how many staff are awake on the unit overnight, how staff are trained in dementia care, how the residence handles falls and hospital trips, what activities run in the afternoon and evening, and whether hospice can be brought in.

Visit in the late afternoon, when restlessness is most common.

Is there financial help for memory care in New York?

Medicaid does not pay SNALR room and board, but the state has offered a limited Special Needs Assisted Living Voucher program for people with dementia, and VA Aid and Attendance or long-term care insurance help some families.

Ask each residence whether it participates in any voucher or Assisted Living Program.

Does Medicare pay for memory care?

No; Medicare does not pay for memory care in assisted living or for long-term custodial dementia care, though it pays for doctor visits, hospital care, hospice and some cognitive assessment and care planning services.

Medicaid pays for nursing home dementia care for eligible residents.
