What does a nursing home provide?

A nursing home provides 24-hour skilled nursing, rehabilitation therapy, help with all daily activities, meals and medical oversight for people who cannot safely be cared for elsewhere, under 10 NYCRR Part 415.

The state roster lists 49 nursing homes in Monroe, Ontario, Wayne, Livingston and Orleans counties with 6,800 beds; CMS rates 45 of them, 22 with four or five stars overall (Care Compare data as of October 1, 2026).

How are nursing homes rated?

CMS rates every certified nursing home with an overall star rating and separate stars for health inspections, staffing and quality measures, and New York requires at least 3.5 hours of nursing care per resident per day.

Our directory shows each home's CMS stars and reported staffing hours against the state minimum.

What does a nursing home cost in New York?

CareScout's 2025 New York median is $15,528 a month for a semi-private room and $16,729 for a private room; Medicare pays for short skilled stays and Medicaid for long-term care of eligible residents.

Medicaid's 2026 regional rate for the Rochester region, $15,675 a month, is the official local benchmark used for transfer penalties.

What should Rochester families ask a nursing home first?

Ask about nurse and aide staffing on evenings and weekends, how many aides are agency staff, the latest inspection findings and plan of correction, and whether residents keep the same room after converting to Medicaid.

Visit at a mealtime and on a weekend. CMS's staffing figures are quarterly averages; what you see on a Sunday afternoon is the real number.

How do short-stay and long-stay residents differ?

Short-stay residents are usually on Medicare for rehab after a hospital stay, while long-stay residents live in the home and pay privately or through Medicaid, and CMS publishes separate quality measures for each group.

Read the measures that match your parent's situation.

How do Rochester families get a nursing home bed quickly?

Most admissions come from hospitals, whose discharge planners send referrals to several homes at once; families coming from home should call admissions offices directly, ask about waiting lists and have Medicaid paperwork started if needed.

Large homes with several units often have openings sooner. Ask about short-stay rehab beds separately from long-term beds.
